What is Risk Register?

The risk register is the living log of identified risks: each entry with its description, probability, impact, planned response, and — critically — a named owner. It's created early and updated throughout, not written once and archived.

The register's real test is behavior: a risk register nobody reviews in status meetings is a document, not a practice. Exam scenarios reward the PM who updates the register and activates the planned response, in that order.
