What is RACI Chart?

A RACI chart maps every activity to who is Responsible (does the work), Accountable (owns the outcome — exactly one per activity), Consulted (gives input), and Informed (kept updated). It exists to kill the two classic failure modes: everyone assumed someone else had it, and five people think they're the decision-maker.

The exam loves the cardinal rule: one and only one Accountable per row. Multiple A's is the textbook error to spot.

Worked example

For "approve the test plan": QA lead is Responsible, project manager Accountable, security architect Consulted, dev team Informed. One A per row — always.
